Below are some of the required forms that Progressive Urban Property Management will use to properly manage your rental unit. For your convenience, our office manager can send you all the forms electronically for you to complete and to get your signatures.
Property Management Agreement
Progressive Urban Property Management Agreement for the Essential or Premium management packages. This agreement will allow us to manage your rental property.
Lease Up Only Agreement
If you’re only interested in having us lease your rental unit, please complete this lease up agreement.
Lead Disclosure
Please complete this lead disclosure form for the property.
Authorization for Direct Payments
Please complete this form so that we can send you rental payments electronically. Electronic payments are an efficient and safe method to receive your rental payments.
W9 Tax Form
Please complete the W9 tax form that will allow us to send you 1099 and other tax reports to you at the end of the year.

What a great group of people. We just signed on with PUPM for managing our rental house and the group is very professional and organized to the hilt. George Rofail was terrific in meeting with us and introducing us to other members of the group. And Amy Pierse, Director of Operations, is fierce!!! She is an extreme go-getter and very much on top of things for transitioning the property from our previous management company to PUPM. Such a professional and great communications as to the status of the transition. A great relief and we are so very happy that we working with this group!!

I was having issues with invoices not being done on time or being done at all, and couldn’t get any definite answers or even a returned call, until I got in contact with Melissa De Leon, she immediately got it done and got me taken care of without any issues. I highly recommend her she put my mind at ease and I know moving forward I won’t have the issues I was having. Thanks Again Melissa De Leon
